107. Treatment of peripheral nerve sensitisation with Dr Toby Hall
When your patient has neck and arm pain, or low back and leg pain from neural tissue pain disorders (NTPD) such as peripheral nerve sensitisation (PNS), how will you treat them? Pain associated with PNS can occasionally be mild and non-irritable, but more often than not, it’s severe, highly irritable, and easy to stir up. How can you provide treatment that settles their pain, without stirring them up? What advice, education, manual therapy and exercise will you provide to help improve symptoms and speed up recovery? What are the keys to success with PNS patients? In Physio Edge podcast 104, Dr Toby Hall and I discussed PNS, common symptoms, causes, questions to ask, and how to assess and diagnose PNS in your patients. In this followup podcast, the second in our two-part series, Dr Toby Hall and I take you through the next phase - how to treat PNS. You’ll discover: * The 7 keys to success with PNS * How to successfully treat PNS in the neck and upper limb, AND the low back and lower limb. * What education and advice should you provide to your patient about activities to avoid or reduce, and which activities should they increase? * What are the most effective exercises for patients with PNS? * Should exercise be painful or painless? * When is neural mobilisation an effective treatment? * When should you avoid using neural mobilisation as a treatment? * Is manual therapy effective in PNS? * Which manual therapy techniques can you use to improve symptoms and range of movement (ROM) immediately? * How to perform effective manual therapy techniques that reduce pain without stirring up your patients. * How can you combine neural mobilisation with manual therapy? * If you use manual therapy to improve symptoms, what home exercises should patients perform after each treatment session? Rob Beaven & Dave Elliott
Core Stability & Other Myths
Dave talks to Greg Lehman, Physiotherapist, Chiropractor and Strength and Conditioning Specialist about core stability and the movement that matters in this episode. They discuss what your core actually is and how you can optimise your exercise and movement patterns to maximise the outcomes that are the best for you Greg also shares great information about our, often hidden, 6 packs the different ways in which we exercise them and how everyone has a good core. This is a great chance to understand more about core stability, being human and movement that you enjoy.   KEY TAKEAWAYS Your core usually defined as your torso and hips and has many muscles. Exercise has multiple effects and exercise for lower back pain can be helpful but does not have to be very specific exercise. Pain doesn’t always mean you should stop movement but you need to move in a way that is best for you. We should move faultlessly and fearlessly; you shouldn’t have to think you should be able to trust your body. Pain is not just mechanical its influenced by many other factors such as fear and worry There isn’t a relationship between someone’s posture and them having back pain There are joints in your body that enable you to move in different ways so its important that you do just that. You can set yourself ‘goal’ exercises that you want to work towards. Exercise is important especially as we age for bone density and loss of muscle mass For returns on time and energy chose exercise that you enjoy doing so you keep doing it. It’s more about being a human and movement that you enjoy doing.   BEST MOMENTS ‘Do whatever makes you  feel better because moving in as many different ways as possible will give you more options’ Most people have a 6 pack, you just can’t see it on everyone’ ‘You should be able to move without thought’   VALUABLE RESOURCES The Back Pain Podcast The Back Pain Podcast recommended products affiliate link OA Optimism website Movement Optimism website Greg Lehman Twitter   ABOUT THE HOSTS Dave Elliot Dave is the owner of Advanced Chiropractic, a chain of Chiropractic and massage therapy clinics in Essex, UK. #151: Steve's worldview, biases & approach to COVID-19 research
This is a shorter solo podcast, for those interested to know more about the producer of all of AdapNation’s COVID-19 and associated content, and how it’s done. This episode is part confessional, part insight into how I work, and part instructional. It’s great to see our content is helpful and valuable to so many in calibrating their knowledge and perspectives regarding COVID-19, human health and government public health interventions. More and more people are gaining the courage and confidence to share our evidence-based content, set up their own pages and even produce their own analysis. Amazing! But, let’s be clear, we ALL have our biases. You can’t be human and not have your own worldview and biases, that inevitably act as both a map and a lens in which you evaluate and respond to reality. I (Steve) am no exception. So, in an effort to be an open book to our supporters, new followers, and even sceptics of our work, I thought it would be only right to share my health-oriented worldview and known biases. Yep, it’s a laundry list! No doubt you would have experienced my biases in my work, so this is merely a confession to the fact I know that they exist and I try my best to mitigate them. No one is immune to cognitive dissonance, me being no exception. The more open we can be about this, the better we can explore truth versus attempting to bend reality to match our expectations and beliefs of the world. This episode will also let you in to how I further develop my my subject matter knowledge, how I research and investigate issues, how I prevent being in an echo chamber, my approach to content creation, the important aspects of statistician analysis and visualisation, and the common COVID-associated data sources AdapNation frequently use in our work.
