According to the Centers for Disease Control and Prevention (CDC), six in 10 American adults have one chronic disease and four in 10 have two or more chronic conditions. Risk adjustment aims to identify and treat these highest risk patients.
Strong patient stratification and care management programs are the best way to close gaps in care, reduce costs, and ultimately achieve better health outcomes. But getting started with risk adjustment isn't always easy — doing it right is even harder.
Beth Israel Lahey Health Performance Network (BILH-PN) began a small pilot program to see if they could make an impact. They took a creative approach to risk in their network by proactively reaching out to get patients in the office and using refined documentation processes to track progress. Their small pilot program is now expanding.

The first 30 days after patient discharge are critical.
According to CMS, nearly one in five Medicare patients discharged from a hospital — approximately 2.6 million seniors — are readmitted within 30 days, at a cost of over $26 billion every year. Transitional Care Management (TCM) aims to deliver proper care to patients and can reduce readmission by a staggering 86%. But disparate data can delay results for healthcare organizations implementing TCM programs.

Episode 1: We didn’t start the FHIR
Data on a single patient can flow from point A to point B. But what does it mean to achieve interoperability at a population level?
As an industry, we have made meaningful progress in interoperability since the passing of the HITECH act in 2009. Use cases like sending an ADT notification or exchanging a CCDA document have largely been solved for and there are exciting new standards like FHIR which have tremendous promise.
There is still a lot of complexity, however, in solving interoperability across a large provider base with multiple EHRs and disparate workflows. Standards can only get you so far and the crucial last mile of interoperability — making data seamlessly available in provider workflows — is not just a technical problem.
Your organization needs the ability to enable providers to act on whole-person insights at the point of care and drive outcomes at scale. That requires a massive rethinking of the problem.
